Family & Systems Teams "helping Maine families achieve their goals through a voluntary collaborative planning process" What is FST? Family and Systems Teams (FST) is a facilitated collaborative planning process that gives Maine families the opportunity to work with a team of providers and natural supports to develop a comprehensive plan to better their lives. FST is safety-centered, strengths-based, and designed to meet the needs of all family members. Feedback from FST participants tells us that initial FST cases have been very successful in helping families make substantive changes in their lives When can FST be used? FST is most useful when a family is involved with numerous systems and not able to access wraparound or targeted case management services Criteria for referral: ~Multiple systems are involved, without benefit of a comprehensive plan ~Two or more agencies are providing services ~The family is experiencing significant difficulty in several of the nine life domains ~There has been long term involvement with multiple systems without significant improvement in quality of life for the family How does FST work? ~An FST Facilitator meets with the family to develop a strategy that brings all relevant people to the table ~Using a process that identifies the family's strengths and needs, the Family and Systems Teams develops a comprehensive plan involving the family, providers, and natural supports ~A Plan Coordinator designated by the Team continues to work with the group to coordinate service delivery and fulfill the plan's objective Who to Contact: FST Project, Institute for Public Sector Innovation, Muskie School of Public Service, University of Southern Maine 295 Water Street, Augusta, Me 04330 Erica Hansen King 626-5046 erica.king@maine.gov Annelisa Schneider 626-5290 annelisa.schneider@maine.gov Referral to FST are through your local Regional Children's Cabinet: Region 1York, Cumberland Dan Harfoush 822-2855 Rachel Posner 822-0246 Frances Ryan 822-2316 Region 2Oxford, Androscoggin, Sagadahoc, Franklin, Lincoln, Kennebec, Waldo, Knox, Somerset Robin Brookes 442-7963 Region 3Hancock, Washington, Penobscot, Piscataquis, Aroostook Judy Fortin 941-4422
