Active employee biweekly rates: Employee only: $2.46, Employee+1 dependent: $3.94 and Employee+family: $6.40
Retiree monthly rates: Retiree only: $5.90, Retiree+1 dependent: $9.47 and Retiree+family: $15.36
Cobra monthly rates: One Person $5.02, Two Person $8.04, and Family $13.06
