The following is a partial list of standard BGS PDCD forms and documents available for State Agency use for ARRA projects. Contract forms have been highlighted in bold print. Please contact PDCD staff for additional information and documents.

| Public Improvement Projects (PIP) Request Form |
All public improvement projects begin with this project initiation form and an approval by BGS. Under Comments, please indicate if ARRA funds are being used in part or in whole for this project. |

Professional Services Agreement - Programming and Schematic Design

This form is used for early Architectural / Engineering project planning services that may or may not result in a full service design agreement at a later date. |
Professional Services Agreement - Limited Scope
Rev. 4/15/09 |
This form covers any number of Architectural / Engineering projects whose phases are described adequately in brief form. The services would include one, or just a few engineering disciplines or architectural services. |
Professional Services Agreement - Consulting Services
Rev. 4/15/09 |
Similar to the Limited Scope form, this form is for non Architectural / Engineering professional services such as appraisers, surveyors, estimators, et cetera, although this form could also be used for Architect or Engineering services. |
Professional Services Agreement - Industrial Hygiene

This form is for these specific services only. Firms that conduct evaluations for, and design the removal of hazardous materials use this form. Companies that perform the actual removal of hazardous materials do not use this form. |
| Professional Services Supplemental Agreement |
This form amends the time, scope, or cost of any of the Professional Services Agreements. |
| Sample Notice to Municipality |
Letter to notify municipality of a state public improvement project in that municipality. This letter is typically submitted as soon as practicable after beginning the schematic design process. |

Link to online site providing wage determinations for construction contract values of $2,000 or more. |
| Sample Legal Advertisement for Contractor Proposals (RFP) |
This document is used when an Agency is ready to procure a Contractor to perform the necessary work required for the public improvement. Please contact your PDCD Project Manager. |

| 2-E Construction Contract - No Bonds |
This form is used for projects with a construction cost up to $100,000, where the Owner Agency has opted to not require bonding. Please contact your PDCD Project Manager. |
| 2-E Construction Contract - Short Form |
This form is used for projects with a construction cost greater than $100,000, where filed sub-bids have not been included. Please contact your PDCD Project Manager. |
| 2-E Construction Contract - Long Form |
This form is used for projects with a construction cost greater than $100,000, where filed sub-bids have been included. Please contact your PDCD Project Manager. |
