This section is designed to assist case managers who are working with people transitioning into Developmental Services. The site is designed to assist with the “nuts and bolts” of referral, eligibility, and accessing services. Case Managers are highly encouraged to look at the links provided to other information and services regarding transition as well as seeking out trainings and informational forums regarding transition to adult services.
Section 21 - Section 21, Home and Community Based Waiver (“Comprehensive Waiver”): The Comprehensive Waiver offers a broad mix of services including Home Support, Community Support and Work Support. Other services are available and may be recommended for participants through the person centered planning process. The waitlist for Section 21 is determined by level of need based on health and safety.

The Support Waiver offers supportive services to consumers who live with their families or on their own. The primary services are Community Support and Work Support. There is also a mix of other services that may be recommended by the person centered planning team. The waitlist for Section 29 is determined according to the date the person is found eligible.
