A Discussion of Person Centered Planning

In a Medicaid /Resource Allocation World

There are a lot of changes in the PCP process lately. Some of them in rule or expected procedures, and some of them in myth only. We hope this workshop will help to clarify the standards that need to be met in personal planning when it is used as the justification for resource allocation through Medicaid.

In Many instances, Person Centered Plans are meeting the standards in the three areas we will discuss. But there are also plans that do not meet these requirements for Medicaid Reimbursement. If agencies were audited for these services, they may find themselves in trouble.

Example : 1300 hours for community services approved, but the only service the plan outlines is to teach the person to take his meds independently.  This is certainly not a service that will take 1300 hrs. to accomplish. What else is being offered to support the person in his community?  What else was talked about during planning? Is taking his own meds really this man’s only need or desire?

There are three processes and all three need to be comprehensively done and documented to produce a plan that meets good ethical standards, and meets the needs for resource allocation. The people primarily responsible for assuring the team meets these standards with the plan are the PCP Coordinator and the Case Manager.
