Adult Case Management Services:
Adult Case Management is a program that is based in the adult consumer's home community. The Broadreach Community and catchment area for services is Knox and Waldo county. Adult case management typically provides advocacy in obtaining the services needed to improve the eligible individual's, and their family's, quality of life. Case managers meet with consumers and their families, developing partnerships with other providers to facilitate coordination and delivery of a comprehensive system of supportive services. They may serve as service advocates, educators and resource coordinators ensuring access to services for identified needs through the development of a plan of service based on the consumer's and family's goals. The plans begin with identifying strengths, empowering each individual to stay safe, have healthy lifestyles and be an active member of their community. Engagement with Adult Case Management is flexible, determined by the individuals ability to learn and to grow, honoring the clients changing needs throughout their lifetime, encouraging independence and self sufficiency.
