Description of Services:
Community Case Management
Case managers meet with consumers and their families to monitor services and assure his/her satisfaction with the process and outcomes of support, services and available resources. The primary role of a case manager is to assist in identifying and implementing support strategies that reflect the consumer’s personal vision. We will assist in the development of a person centered plan followed by the monitoring of goals and objectives in order to meet your needs and desires.
