Description of Services:
Group Home Foundation was founded in 1976 to provide supports to adults with mental retardation. It's mission is to provide supports for living and working in the community. The services offered include residential facilities licensed by the State of Maine, and employment and day habilitation services that are nationally accredited (CARF). A supported living program provides individualized community based supports within the individuals' own home.

Building upon the years of experience providing individualized planning Group Home Foundation has become certified by the Department of Behavioral and Developmental Services as a provider of case management services (CCM) for adults with developmental disabilities. These services will be offered in the Northern Mid-Coast area (Knox, Waldo and Hancock counties). Referrals for individuals outside of that area will be considered on a case-by-case basis. The case manager will work with the individual and team members to develop individualized plans that identify needs, resources and linkage to coordinate and monitor them. Referrals may be directed to the individual listed above.
