Description of Services: The Adult Community Case Management (CCM) program of Independence Association provides comprehensive case management services to Adults with Developmental Disabilities. Our goal is to support people in achieving a full and inclusive life in their chosen community. We accomplish this goal by developing Person Centered Plans with meaningful and attainable goals based on the individual strengths, needs and desires of the person we are supporting. Our goal is to help people use their strengths to meet their own needs. Coordination of necessary services, advocacy, referral and monitoring of services to ensure success are just a few of the ways that we provide our support services. Home, program and community visits are an essential part of the service. We strive to develop a system of natural supports to help people achieve independence.
The Community Case Management Program operates in the greater Brunswick area which includes all of Sagadahoc County, and portions of Cumberland, Androscoggin and Lincoln County.
Eligibility for case management service is determined by the Office of Adults with Physical and Cognitive Disabilities. Referrals may be made directly to the agency at the above listed contact numbers.
