Description of Services:
Case managment services provide for the coordination of services to adults with cognitive and physical disabilities and their guardians. Services include meeting people to discover the person's/family's needs, developing individualized service plans to address needs, advocacy to management services are provided through office visits, home visits, and a variety of other community locations where adults need assistance and coordination.
Service Outcome:
Adults receive coordinated supports to meet their unique needs; Adults are supported in ways that allow them to be successful in their homes and community.
