Description of Services:
 Adult Case Management is a program provided by Northern Maine General to individuals who reside in Aroostook County.  It is offered to Individuals over the age of 18 who have been diagnosed with mental retardation, pervasive developmental disorder, and/ or autism. The Case Management program offers an array of services which includes but is not limited to the following;

• Representative Payee
• screening and assessment
• individualized service planning
• Person Centered Planning
• service linkage and service monitoring
• assisting with accessing funding for services

Case Management also helps individuals address their needs to improve overall functioning.  Case Managers provide support to assist individuals with locating and accessing resources to enhance their growth and development. An Individual Support Plan (ISP) and/ or Person Centered Plan is developed which allows the multiple needs of each individual to be addressed in a cohesive and complementary manner.
