Description of Services:
One of the primary roles of a Community Case Manager is to assist and support the individual
receiving services with planning their lives. The Community Case Manager needs to assure each
individual that they support, receives an opportunity to participate in personal planning. Community
Case Managers enjoy a very unique relationship with the individual, a relationship that is in
large part determined by the person's personal vision for a desired life. For some individuals,
the relationship needs to be supportive and non-directive, for others, the Community Case Manager
needs to assert more responsibility. The foundation of the relationship between the Community
Case Manager and the individual lies in the role of assisting the person to reach their vision
for a desired life and monitoring the quality of their life. Monitoring is done through personal
contact and phone calls to the individual, his/her family, and involved support staff. The frequency
of this contact is determined by the individual's needs, requests, problems, type of program
and the personal planning process."~ We also provide Representative Payee Servcies.
We provide Case Management Services in Androscoggin, Franklin, Oxford and Kennebec Counties.
