Description of Services:  
Penobscot Community Case Management is a community-based program provided to eligible individuals 18 and older who are diagnosed with a developmental or intellectual disability. Areas served are: Hancock, Penobscot and Washington Counties. Community Case Management services assists individuals and their families to design and implement strategies for obtaining services and supports that are goal-orientated and individualized. Services include assessment, planning, linkage, advocacy, coordination and monitoring to assist individuals in gaining access to needed health and dental services, financial assistance, housing, employment, and other services and natural supports developed through the person-centered planning process. Additional services may include SSA Representative Payee (for individuals receiving services from Penobscot Community Case Management).

Penobscot Case Managers perform their work using the principles of freedom, self-determination and civil rights as their guide. Our Case Managers recognize the significance of the individual’s family and approaches to service delivery within this context. Our Case Managers are caring professionals who listen, advocate, support and coordinate services with families and community providers. Each Case Manager is trained to utilize all avenues of person-centered planning to best meet the unique needs of each person we support.
