Description of Services:
Adult Community Case Management links a person and their family with services throughout York and Cumerland Counties. We will advocate for a consumer and their family, monitor services, hold the annual Person Centered Planning Meeting and provide support as needed. Two home visits are required per year and two program visits. Case Managers will offer guidance through issues such as Mainecare, Social Security, Work Incentives Coordination, Vocational Rehabilitation services, section 29 and section 21 planning, nursing care if needed, medical , dental, psychiatric needs.
