Description: Community Case Management Services provided to eligible individuals 18 and over living in Kennebec or Somerset County. Case managers assist in the development and implementation of Person Centered Plans. They can assist in accessing resources and available funding for identified needs. Youth & Family Services also provides case management to children with diagnoses of mental retardation or mental health issues.
