Scenario: In this section of the lesson, Laurie and her family have been matched for an assessment period with Care and Comfort. The QIS Staff person has submitted a request to the department for a change of status. This needs to be documented with a note on the client tracking record.

Step: Click PROCESSES in the EIS Menu "Hot Area" User Prompt: You have already anchored on Laurie. Access her processes.

Step: Click underlined process NAME "Hot Area" User Prompt: Access her CE process (CE/R2/65M).

Step: Click #4 - CREATE CLIENT TRACKING "Hot Area" User Prompt: Access the client tracking record found in Step #4.

Step: Click SCROLL down bar "Hot Area" User Prompt: Scroll down to access the client tracking record.

Step: Click underlined Client Tracking ID "Hot Area" User Prompt: Access the client tracking item.

Step: Click ADD button "Hot Area" User Prompt: Add a note.

Step 1: Type CE/R2/Review Care and Comfort in the TITLE field. Step 2: Type 06/15/2005 in the END DATE field. Step 3: Click ORGANIZATION select Hyperlink User Prompt: The title is CE/R2/Review Care and Comfort and the date is 06/15/2005 (which is the date the client was authorized for assessment). You also need to attach this note to the provider.

Step 1: Type care in the QUICK FIND field. Step 2: Click GO button Hyperlink User Prompt: Find an organization with 'care' in the name.

Step 1: Select (25432/32573) CARE AND COMFORT/WATERVILLE / 180 MAIN STREET from the [list box name] list box. Step 2: Click OK button Hyperlink User Prompt: You want (25432/32573) CARE AND COMFORT/WATERVILLE / 180 MAIN STREET.

Step: Click NEXT button "Hot Area" User Prompt: Continue.

Step 1: Select CS CENTRAL ENROLLMENT NOTES(CLIENT TRACKING)(Ver 2) from the TYPE list box. Step 2: Click FINISH button Hyperlink User Prompt: The type you want is CS CENTRAL ENROLLMENT NOTES(CLIENT TRACKING)(Ver 2). And you're done starting the note.

Step 1: Select BEHAVIORAL CONSULTATION from the DOMAINS list box. Step 2: Click SELECT button "Hot Area" User Prompt: Assign BEHAVIORAL CONSULTATION as a domain.

Step: Click SCROLL down bar "Hot Area" User Prompt: Go to the bottom of the note.

Step 1: Click WAITING in the Status Level check box group Check Box Step 2: Select CHILD/FAMILY AUTHORIZED FOR ASSESSMENT PERIOD from the WAIT IN CENTRAL ENROLLMENT drop-down box drop-down list. Step 3: Click SCROLL up bar "Hot Area" User Prompt: You are still WAITING, only now the CHILD/FAMILY AUTHORIZED FOR ASSESSMENT PERIOD. After documenting this, go back to the top of the page.

Step: Click SAVE button "Hot Area" User Prompt: Save the note.

Step: Click CLOSE button "Hot Area" User Prompt: If needed, you could now click the ADD DESCRIPTION button to add further text to the note, we do not need to do that. Go back to the process.

Step: Click CLOSE button "Hot Area" User Prompt: Go back to the process.

Step: Click CLOSE button "Hot Area" User Prompt: Go back to the process.

Step 1: Select 7. CREATE AUTHORIZATION FOR CHILDREN'S SERVICES from the PROCESS STEP drop-down drop-down list. Step 2: Click GO button "Hot Area" User Prompt: Go to Step #7 of the process.
