1. Duplicate Forms. Please do not fax and then send copies of materials.
Do only one or the other. Processing the originals is happening and then
copies are arriving and confusion is occurring.
2. Exams: You do not need to send me a copy the student's exam unless
requested by the Department. Only a copy of the exam used is necessary.
Please remember that even with the 3-day course all the requirements as
outline in the 5-day course are applicable. An exception to this is that you
will have to create your own exams from the course you are using.
3. Some instructors are sending in more information than necessary. The
packet arrives with the first validation form checked, student information
forms, copy of exam (s), and evaluations forms (do not send separate
validation form for each student information form). You need to make sure
that ALL student information forms are in that packet.
The second Validation form (will have both boxes checked and a listing of
the student's that certificates can be issued. If you are holding for
funding, when you send the second validation form with their name, that will
indicate that the certificate can be issued, (you do not have to send
another copy of the exam (s)). You have already done that with the initial.
Then sometimes when a clinical and second validation arrives, some
instructors are including another copy of exam (s); this does not have to
happen. Only the validation form with both boxes checked and the clinical
checklist needs to be sent. Remember to have the name of the trainer and
date of class on each clinical checklists before the student takes that to
the clinical person.
4. Remember only curriculums previously certified by the department may be
used in the 3-day course. If you are adding, deleting or changing anything
you only need to send the materials you are adding or changing to the course
(with the source identified) and a listing of the material you are deleting.

5. As you folks are the first line of defense (sort to speak) it is up to
you folks to track the 30 day clinical period. I have no objection to
granting an extension for a valid reason. If a student has gone well over
the 30 days and you have not requested an extension, then you must let me
know who that student is so I may process a "did not complete" and file the
packet.
6. Student information forms have lacking information. Some students
neglect to put the city they live in, lots of zip codes are missing, and
they are not writing legibly. The student's home address must never be the
work address. I need the home address for the database.
