Please be sure to be using the new Validation form that has the two
lines of information needed. Date and Completed by.

Also, after the class, you do not need to send in any paperwork. You
can send in the validation form with the student information sheet as
you receive their med passes. You can send in the evaluations with any
of the validation forms.

For 24 hour certificates, you need to determine if it was a CRMA or RN
doing the med passes. This could jeopardize the student getting the
correct certificate.
