Instructors: Forms have been updated with changes recommended by
instructors and updated.
Class Participation Checklist:
This form has been added to the web site for easy access. This form is
for 25% of the grade for participation in class for the 40 hour
curriculum. The instructor selects a date at midpoint of the class with
the student to go over the scores and at the end of class. The student
may not agree but has input on their own perception of participation.
The instructor checks each item below under date with a score of 1
through 4 for each item below and totals the score at the bottom:
1. Timeliness
2. Group participation
3. Respectfulness and Listening abilities
4. Offers and articulates comments and criticisms
5. Initiative with tasks in class
6. Completion of homework and class work

After the second meeting with the student, the score is written again in
the space provided. Both scores are added together from space one and
space two then entered into the web grading grid for a final % score.
Call me if this is not clear. This tool can be very useful for screening
students' behaviors and classroom performance. This is not an optional
form. It is part of the grading of students. There have been incidents
instructors have brought to my attention; if this tool had been
completed as designed would have assisted the instructor in difficult
situations but without this completed; I have no ability to support you
as an instructor.
The third page of the medication administration skills checklist is very
useful for details of the students' performance of the recertification
medication pass. It can be used as student documentation to show
retained knowledge of page items not performed (i.e. storage of
medication or receiving and counting medication). This is also useful
for the instructor's comments of good/poor performance during the
classroom 6 medication passes.
