Hi Instructors-
Peter and I met today regarding the PSS forms. Revisions on the forms and
adding the new department name will be made. Keep a copy of this e-mail for
reference.
1. The Grid will have the spelling corrected in class participation.
2. The Skills checklist will have a wider space for the students name,
date, and completion date.
3. The Notification form will have a line added to include the type of
training.
4. The Required Process form page one, will have the program name
changed, and #1 and #2 will include rewording.
5. The Required Process form page two, will have acronym names changed
and wording changes. Copies of exams and quizzes are no longer required to
be sent in.
6. The Trainer form will only need to be sent when there are changes in
name, address, phone, e-mail etc. Only the last four digits of your social
are required. Please remember to put your mailing address not physical
address on the trainer form.
7. The Validation form where the boxes are will be reworded. Please put
all student names in class that passed on the Notes/Comment line for ease of
processing. The validation form can be faxed instead of mailing.
8. The Student information form will indicate only the last four digits
are necessary for our database so that students and providers may get
additional copies of certificates.
