Instructors- Please look one more time, I have looked until I can't
recognize errors anymore. I believe now all forms are currently updated
with address changes and spelling corrections. Let me know if you find
anymore, David Perkins made final changes last week after input from
many of you. Thanks again, for your time. All forms are corrected and
changed.

When you send in your notification forms please do not forget to put in
your type of training as the clerical help can not figure out from a
single date whether it is a Bridge or Recert. Thanks.
