Hi instructors,
1. When you send in your initial classroom completion packets, you may
send your actual attendance sheet at the same time as many of you have drop
outs or add ons prior to or on day one.
2. There is no need to send in the trainer form and copy of license
with every notification, just at time of license renewal or with changes of
phone or address. Be sure to highlight or mark in some way on the front of
the notification that there is a change for the data entry person to
recognize or they may not see there is a change.
3. If you are getting letters from us on students that have not
finished their clinicals in the 30 day window you may want to remind them to
get them done at an earlier time frame or when completion occurs Fax the
validation and med pass in and you keep the original. That way you have the
original should it get lost, it gets here faster, and we have a copy of the
original validation from the first packet.
4. The following instructors are available to do passes for students if
