I'm not sure what the questions are about, is this on the new CRMA curriculum? In any case, if I received the second order: "q 4 hours. and may have two additional doses for severe agitation for a maximum of 8 doses in 24 hours" I would be questioning the doctor about the time frame for the additional doses. This order makes it sound like the resident could receive a scheduled dose and one minute later an additional dose and one more minute later another additional dose.

There have been questions about the transcription of the xanax order. The regulations require that antipsychotics be given on a scheduled basis. Psychotropics such as xanax may be given PRN as long as there is a maximum number of doses specified. If the frequency is "q 4 hrs PRN." the maximum in 24 hours would be six doses based upon the frequency identified. If the doctor orders
"q 4 hrs. and may have two additional doses for severe agitation for a maximum of 8 doses in 24 hours," the maximum dose in 24 hours in 8. (The first one specifies based on the frequency, the second is adding to the frequency additional doses if needed.)
