beas-crma Curriculum Information

Please review student information forms for completeness as we have had to
call in order to get information to mail certificates. We will not be
calling students anymore. We have several certificates of students who have
given incorrect addresses that have come back in the mail.
 
I have met with Peter Mauro Asst. Director to discuss changes in CRMA
curriculum forms. Peter will be working on the forms, then sending them to
our web master to be added.
 
Paper flow changes will be as follows:

1. The CRMA process form will be rewritten to include the below changes
and #1. on page one wil be reworded to say the Department may generate a
report....
2. The CRMA checklist will also be rewritten to reflect the below
changes. The double asterick sentence will say original signature can be
faxed.
3. Notification forms may come in 30 days in advance of classes or
more. If the class is full you may send the form in later, as long as it
comes to our office before the first day of class.
4. The student information form and trainer information form will have
the licenses and certifications line removed and the social security will
allow for a complete social security number if the student wishes but
indicate at least the last four digits are necessary for our database.
5. The Trainer form will be sent in when there are changes of address,
phone number, name etc. and a copy of RN license must be sent in upon
renewal of license.
6. The evaluation form will stay as is with only changes to the Dept.
title.
7. The validation form bracketed boxes will be changed to a simpler
phrasing.
8. The Classroom Roster does not need to be sent in. This will be tried
for 6 months. If we get calls from alot of students saying they were in a
class and we can't issue a cert; we may go back to the roster. Please list
all students completing the class on the Validation form. The reason being
that we had three students that paid and paperwork never came in so we had
to tell them without proof we couldn't issue a certificate. The roster was
our only proof.
9. The blank final exam does not need to be sent in with your classroom
packets. This will be tried for 6 months. If we get reports of instructors
altering the final exams we will return to our previous method. Please
adhere to the exams that go to the particular standardized curriculum. They
are not interchangeable. We have seen Bridge exams coming in for the Recert
class exams.
10. Please let Peter or myself know if this is improving the paper flow
process. Keep this e-mail for reference until all the forms have been
