Some of the clinical instructors are not sending in a validation form with
the clinicals. We are having difficulty identifying the instructor and class
for clinicals. Could instructors please contact these nurses and provide
them with completed validation forms or have then return the clinicals to
the instructor to send. Some student have been sending them in as well.
