Since my e mail this morning, I have spoken to a few providers and discussed
with them the need to put all PSS persons on the database. I realize that
before we might have been going in a different direction, but with the
advent of EIM's question we need to move into this other direction.
I appreciate your cooperation and understanding:
1. If the CNA is still active on the registry - simply fax or send me a PSS
student information form with the date for the certificate, and your name if
your a PSS instructor. If your agency is utilizing the skills checklist
please send that along also. (not required)
2. If the CNA has lost his/her standing on the registry then the final exam
from the PSS test bank, a student information form, and the skills checklist
(if utilized by your agency)
Certificates will be mailed directly to the employee.
