To become a DEEP-certified community-based service provider:

Agency – contact DHHS licensing at (207) 287-4399

Private Practice – contact Ann Levesque or Bill Crane at 207-626-8600.

Provider training opportunities:

DEEP will be holding its Fall Regional Provider Meetings in October of 2008 in Augusta, Caribou, Bangor and Portland. These meetings are required yearly for each provider. This is a one-day training, and is presented by the DEEP staff in partnership with other OSA staff members.

Current DEEP providers will be receiving registration information in the mail in early September of 2008. The registration form may be faxed or mailed to the DEEP office. Once you recieve this information, register quickly as space is limited.
