All public and private programs that intend to provide special education and related services for children under school age five must obtain approval through the CDS State IEU. The program approval process must be completed prior to a Regional Site entering into an agreement for services.

- Program Approval Update Form (DOC, 100KB). This form must be completed and submitted to the CDS State IEU when any change occurs within an approved program.
- Staffing Chart (DOC, 93KB). This form must be kept current and provided to the CDS State IEU when there are staff changes. All staff changes must be provided to the CDS State IEU within 30 days.
