RE: April 1st Resident Enrollment Report (EF-M-12) and Reimbursement for Students Enrolled in Equivalent Instruction Programs (EF-M-12E)
The April 1st Resident Enrollment
Report (EF-M-12) and the Reimbursement for Students Enrolled in Equivalent
Instruction Programs Report (EF-M-12E) are now available on the Department of
Education’s Web page. These reports
must be completed as of April 1 and returned no later than April 15, 2001. These forms may be downloaded from the
Department of Education’s Forms Page at:

If you have difficulty downloading these forms, please call Patrick Dow at 624-6790 for assistance.
For those using the Microsoft Excel program, formulas have been embedded in the forms to automatically calculate the sub-totals and totals. Supplemental pages providing additional space for certain items are included in the forms. These are clearly marked in the Excel workbook. If you need to use a supplemental page, please be sure to use the correct page for the appropriate section.
NEW – The Department is
discontinuing the use of forms EF-M-11/12a - NOTIFICATION OF
TRANSFER OF STUDENTS TO ANOTHER UNIT and EF-M-11/12b - NOTIFICATION OF
ACCEPTANCE OF STUDENTS TRANSFERRED FROM OTHER UNITS. Replacing these forms is a new section, Item 9, on the EF-M-12
April 1st Resident Enrollment Report, for reporting students transferred from
one school administrative unit to another school administrative unit under 20-A
M.R.S.A. §5205(6)(A).

Please indicate in item 9a the number of
students that have been transferred from your school administrative unit to
another school administrative unit.

Please indicate in item 9b the number of
students transferred to your school administrative unit that are included in
item 1 and item 7(SADs/CSDs only).
About
the Reimbursement for Students Enrolled in Equivalent Instruction Programs:
The
statute 20-A M.R.S.A. §5021(8) entitles a school administrative unit to receive
state subsidy for any student who receives instruction through one or more
on-site academic courses from a public school but is not a full-time
student. Students are eligible if they
are participating in equivalent instruction programs pursuant to 20-A M.R.S.A.
§ 5021 and approved for equivalent instruction (home instruction) pursuant to
20-A M.R.S.A. § 5001-A, Sub-§ 3(A)(3).
A school administrative unit that sends any “equivalent instruction
program” tuition student to another school administrative unit or to a private
school approved pursuant to chapter 219 is also entitled to receive state
subsidy under this statute. Please
report your school administrative unit’s eligible resident equivalent
instruction students on the EF-M-12E form.
