SUBJECT:       EF-S-07 Request for Subsidy Allocation of Tuition and Board for Students with Disabilities

 

The Request for Subsidy Allocation of Tuition and Board for Students with Disabilities (EF-S-07) is to be used to report all students with disabilities who have been tuitioned to another school administrative unit, public regional/private day program or private school during the 2003-2004 school year.  This form is available on disk upon request, or can be sent via e-mail to you, or is available on the Department's web page at http://www.maine.gov/education/forms/specservices.htm  However, we still need the form in hard copy, signed by the Superintendent of Schools, to be returned to the Department.

 

Please be sure that total expenses are reported, and the total tuition and board agree with the Total Tuition column on page 16 of the EF-M-45.  If you have paid for treatment for a student in residential or day treatment, include this cost in the "board" column.

 

The EF-S-07 form is due July 31, 2004.  If you have any questions regarding this form, please contact the Office of Special Services at 624-6650.
