OCTOBER/APRIL 1ST RESIDENT ENROLLMENT (EF-M-13/14) PRIVATE SCHOOLS

 
 I N S T R U C T I O N S

 
 
ITEM 1 - Report the number of pupils for each school unit and grade for whom any Maine municipal funds are included in the tuition paid to this school. Report the number of pupils received from each school district (city/town/S.A.D/C.S.D – not Union) on one line (treat each line as a separate school unit). Please consolidate all students from member towns within a S.A.D. or C.S.D. on one line. However, each individual town within unions must be reported on a separate line.

 Please report all students who are state agency clients or state wards on one separate line, and write 'State Agency' in place of the school unit name.


ITEM 2 - Report the number of pupils for each city/town and grade for whom all tuition is paid by other than municipalities or districts (parents, scholarships, charity, etc.) Report the number of pupils received from each city/town on one line (use only one line for each city/town). Report here pupils from the State of Maine only.


ITEM 3 - Report the number of students for each grade who are received from outside of Maine.
