NOTE: To download a copy of the reporting forms below with the calculated fields, right-click on the appropriate Excel file and "Save Target As" or "Save Link As" to your computer for completion.  Return completed form as an email attachment to GPA.DOE@maine.gov. Email is preferred but you may send it on a disk via regular mail to MIS Team, Maine Department of Education, 23 State House Station, Augusta, ME 04333-0023.
