MLTI III MacBook Request



The MLTI III MacBook Request Form is to request MLTI MacBooks from the Enrollment Pool to:

  • Complete long-distance transfers between schools.
    • Schools will continue to communicate with each other to move a device from the transferring school to the receiving school; however, if the schools are far away from each other, making it inconvenient for a physical transfer of the device, the receiving school will now contact the MLTI project office through this online request form. MLTI will help the receiving school determine if the they should contact a local enrollment pool or if the MLTI Project Office should ship a device from the DOE (Augusta enrollment pool).
  • Supplement a school that is in need of a device for a student or teacher who is new to MLTI.
    • The school will contact the MLTI project office through this online request form, and MLTI will determine if the school should contact a school in their area, or if MLTI should ship a device from the DOE enrollment pool.

Please use the form below for EACH MacBook you need to request.

If you have more than five (5) requests, please email the MLTI Project Office with all of the applicable information in the form below and include the number of devices needed for each type of request and the reason for each request (e.g., “5 - 7/8 Grade Student Devices for New Students and 2 – 7/8 Student Devices for In-State Transfers”).

Reasons:

  • Student In-State Transfer – Choose this reason when the schools are located far away from each other. The school receiving the student should be making this request.
  • New Student – Choose this reason when your school has a new student who has transferred in from a private school, home school, or from out-of-state.
  • New Faculty/Staff – Choose this reason when your school has created a new eligible faculty position.
  • Existing Faculty/Staff – Choose this reason when you are requesting a device for an existing eligible faculty or staff member.
