Please use this form to request one of our Media Services: Video conferencing, CD-DVD duplication , and see more below. Be sure you have read about each service and if there is a related cost. Please pick up and drop off equipment between 9am and 3:30pm if possible. ( if no one is in Media Services, please stop in the Library Development Office)

**Be sure to add your date and time when requesting video conferencing. Please include additional information, especially if requesting more than one of an item. Let us know whether you are supplying your own CDs or DVDs. For small orders we prefer you purchase ours to ensure the best quality.
