Do the Online Form 486

• Go to: http://www.sl.universalservice.org/menu.asp
• Under Form 486, click on "Create Form 486"
• Enter your Billed Entity Number (BEN) to start a new Form 486, then click the Find button
• Verify name and address information
• Click Start New Form 486 button
