HealthChoice provides comprehensive coverage. Most benefits are subject to a front-end deductible. Mental health and substance abuse services are also subject to coinsurance. It utilizes the Blue Cross hospital and Blue Shield physician networks.

Blue Choice is a non-gatekeeper HMO point of service product. Most benefits are subject to a front-end deductible and coinsurance. Blue Choice utilizes the Blue Cross hospital and HMO Maine physician network. The Hospital Network Option utilizes a limited hospital network.

Services received from network providers are paid at the highest level of coverage.

HMO Choice is a gatekeeper HMO point of service plan. It utilizes the HMO Maine network.

For most benefits, the highest level of coverage is provided if care is performed or authorized by a primary care physician. Members can self-refer but benefits are subject to deductible and coinsurance.

HMO Maine is a traditional gatekeeper HMO plan. It utilizes the HMO Maine network.

For most benefits, coverage is available for services performed or authorized by a primary care physician.

Group Companion Plan provides non-standardized supplemental coverage for individuals enrolled in Medicare Parts A and B. Benefits are available for Medicare deductibles and coinsurance as well as additional services not covered by Medicare. The additional services are subject to a front-end deductible and coinsurance.
