Minutes of April 28, 2006 Public Board Meeting

 

Members Present:

  1. CALL TO ORDER
    Robert Barnes, Chair, called the meeting to order at 9:15 a.m.

  2. OLD BUSINESS

    1. APPROVAL OF MINUTES

      In a motion by Bethel Shields, seconded by Paul Fredette, the minutes from the April 11, 2006 meeting were unanimously approved as written.

  3. NEW BUSINESS

    1. LICENSURE SUSPENSION HEARING - #FUN-280
      The board met in public session for the purpose of determining whether grounds exist to take disciplinary action against the funeral practitioner’s license of John A. Lancaster.

      After public deliberations and a consideration of all evidence, a motion was made by Robert Barnes, seconded by Paul Fredette, that John A. Lancaster’s license as a funeral practitioner be revoked.Mr. Lancaster shall pay the costs of the hearing and shall pay a fine of $1,500. All in favor with Hervey Clay recused.

    2. LICENSURE APPLICATION REVIEW OF LEONARD PERKINS
      The Board reviewed the application of Leonard Perkins for licensure as a funeral practitioner and his request to sit for the state written and practical examinations.Upon review, in a motion made by John Dunn, seconded by Bethel Shields, the board voted unanimously to deny Mr. Perkins’ application pursuant to the requirements found in 32 M.R.S.A. § 1501 and Chapter 4 of the board’s rules for the following reason(s):
      1. Failure to meet the educational requirements contained in Chapter 3, and
      2. Failure to provide the board with documentation that a minimum passing score of 75% on each of the major sections of the National Conference Examination.

    3. APPROVAL OF APRIL CONTINUING EDUCATION PROGRAMS

      In a motion by Paul Fredette, seconded by Bethel Shields, the Board voted unanimously to approve the list of continuing education programs as reviewed by the Continuing Education Committee at the April 11, 2006 meeting.

    4. ADJOURN

      There being no further business, in a motion by Bethel Shields, seconded by John Dunn, the meeting was adjourned at 2:45 PM
