INSTRUCTIONS FOR COMPLETING SCHEDULE B

Please complete the information pertaining to Registrant Name and Account Number.

Mileage Year. For renewals the mileage reporting period is the last complete July to June for all power units registered in the IRP during the mileage year. A registrant with less than one-year actual operation must submit their actual mileage for the partial year. A registrant with fewer than 60 days actual mileage in the reporting period may estimate mileage again without penalty. If this is a new account you must estimate mileage anticipated in the next year. Estimates must be fully explained.

Prorate. Please show "YES" if the state or province needs to be on your cab card. Please show "NO" if you have miles for last year but will not be traveling there this year. (Please note that we have "NO" already in non-participating jurisdictions.)

Actual Mileage. Please show total actual mileage for ALL power units registered in the IRP during the mileage year for each jurisdiction traveled in.

Estimated Mileage. The first time you register in the IRP, or if you are adding additional jurisdictions, you may estimate mileage. Estimates must be reasonable, and must be fully explained. Please refer to your IRP manual for assistance or call the IRP Unit.

Second Year Estimates. Registrants who estimated miles in a jurisdiction and did not accrue any miles there in the first year may estimate miles for a second or subsequent year. However, second or subsequent year estimates are not included in total fleet miles and will result in registration fee payments in excess of one hundred percent (100%).

Subtotal Estimated Mileage. Please total all estimated mileage for all power units registered in the IRP.

Subtotal Actual Mileage. Please total all actual mileage for all power units registered in the IRP.

Total of All Mileage. Please total estimated mileage and actual mileage for all power units registered in the IRP.

Estimated Mileage Explanation. In the area provided, you must give an explanation for Estimated Mileage.

Signature. Please sign the Schedule B, show your Title within the registrant’s business (if other than officer of the company or owner, a Power of Attorney must be filed) and date the Schedule B.
