Error Report - presented in a database export delimited format. The file naming convention is as follows: excel_error.insurer name.MMDDYYYY.#### (take the FR file name, strip off the FR and add “excel_error.insurer name”). Field alignment listed below.

1. Columns are delimited by “^”.
2. SSN doesn’t contain “-“ i.e. has a format of “999999999”
3. Injury date contains “/”
4. Insurer File # : is field # 21 on the WCB-1 Claim form.
5. If a Claim is processed with no errors, there will be no data for columns
J,K or L but there will still be column separators.
6. If a Claim has more then one error, there will be a row for each error. Columns
A-I will repeat the same data in each row.
