Prior Authorization (PA) Requests for Hearing Aids Reminder
Providers who submit PA requests for hearing aids must complete your submission in one of two ways:
- One way includes completing the Prior Authorization Supplemental form (MA-56 form), which can be accessed by logging into your Trading Partner Account in the Criteria Sheet section of the Health PAS Online Portal.
- You can also include the same information from that form documented within the request. This information should include:
- Hearing aid manufacturer
- Hearing aid make and model
- HCPC code that corresponds with the hearing aid make and model
In accordance with MaineCare Benefits Manual (MBM), Chapter II, Section 60.02-12, hearing aids must be purchased from a licensed audiologist or hearing aid dealer & fitter, utilizing a vendor contracted with the State of Maine’s Division of Procurement Services found on the State of Maine Contracts for the Procurement of Hearing Aids webpage.
PA requests that do not include all required documentation will be deferred. PA requests submitted with information that does not match the procurement contract will be denied.
Additionally, invoices are still required when the claim is submitted.
