If selected to attend this training, are you interested in volunteering to consume alcohol for the purpose of performing SFSTs? This is completely optional and more information will be provided about this at a later time. Anyone who volunteers to drink alcohol must have transportation to and from the training. Alcohol will be provided and measured by SFST instructors. Volunteer drinkers will also need to sign a liability waiver.

PDS will provide lunch at this training. We will make every effort to accommodate common dietary restrictions. Do you have any dietary restrictions?

I certify that I have reviewed the agenda for this training and, if selected, I will attend all instructional time. I understand that space is very limited and that PDS incurs food costs for each person who registers to attend. If I am unable to attend, I will immediately notify PDS via email to darcy.fisher@maine.gov so that someone from the waitlist may be invited to attend.
