Statewide Community Survey

The Maine Shared CHNA wants to hear from people living in Maine about their health and well-being both individually and for the communities where they live. This is one part of our community engagement effort, which also includes focus groups and key informant interviews.

Goal: The survey will be used to validate key themes from our other community engagement initiatives and to gather additional information from a wider array of people in Maine about their health and well-being and that of their community.

Target Audience: Any person, aged 18 and older, living in Maine is eligible to take the survey, including professionals who work in public health, health care, social services, and related fields.

Dates: The survey will be closing on July 5th at 5pm.

How to Take the Survey: The survey can be taken on-line (links provided below) or by completing a PDF version, using the fillable form or printing it (provided below). Printed surveys can be scanned and emailed to: info@mainechna.org or mailed via postal mail to: Crescendo Consulting Group, 63 Federal Street #1028, Portland, ME 04101.

The survey is available in 8 languages – English, Spanish, French, Portuguese, Somali, Lingala, Chinese, and Arabic. When taking the on-line SurveyMonkey version, respondents will be able to select the language they want to take the survey in prior to beginning the survey. Those taking the PDF version can complete the version in the language of their choosing.

People who complete the survey are eligible to enter a raffle to receive one of 10, $100 gift cards. If taking the survey as a hard copy or electronically, surveys and gift card information should be returned separately (see the 2024 MeSCHNA Survey Dissemination Plan for more information).

Help with Distribution: If you or your organization are interested in helping distribute the survey, please contact info@mainechna.org and refer to the 2024 MeSCHNA Survey Dissemination Plan. Please note if you are collecting surveys on behalf of clients/patients/consumers and returning them by mail, they must be postmarked by June 28th.

Flyers are available in the 8 survey languages and in formats compatible with Facebook and Instagram.
