The Maine Migrant Education Program (MEP) provides the following School Survey to all Maine school districts to help the Maine MEP locate families that may qualify for related services. Families can elect to complete this form in their primary language.
Each school district is responsible for sending out a MEP School Survey for every student to complete every year as part of their school registration packet.
If all three (3) questions are answered "Yes", a copy of the survey is sent to the DOE for follow up and the survey is kept on file in the student's records. Schools should shred forms that are answered “No”. The local MEP Regional Coordinator follows up with all families for whom we receive a School Survey to determine eligibility for the MEP and provide supplemental services based on their individual needs.

Community Survey
The Maine Migrant Education Program (MEP) provides the following Community Survey to Maine Community Organizations to help the Maine MEP locate families that may qualify for related services. Any community organization in the state of Maine can include this survey as part of their onboarding, registration or enrollment packets. If all three (3) questions are answered "Yes", a copy of the survey is sent to the DOE for follow up. The local MEP Regional Coordinator follows up with all families for whom we receive a Community Survey to determine eligibility for the MEP and provide supplemental services based on their individual needs.
