Include in your request:
- your name,
- date of birth,
- current address, and
- the type of information you are requesting.
If you require a copy of the letter showing the requirement or judgment suspension, include the effective date of the requirement or judgment suspension along with a fee of $5.
If you wish the information to be faxed back to you, please include the fax number and an additional fee of $2.
